PMDC just restated the hard rule: no ECFMG-listed school, no NRE — and Afghanistan is named

If you're evaluating an MBBS abroad, this isn't only about whether a university is "real" or on a consultant's brochure. It is about whether you will be allowed to sit Pakistan's licensing exam — and register — after you graduate. What just landed (31 July 2026) PM&DC issued Public Notice No. PF-5-ADR-PM&DC/2026/1220 reiterating a Council decision of 24 July 2025: Pakistani nationals who obtain a primary medical or dental qualification from a foreign institution are eligible to appear in the National Registration Examination (NRE) only if that institution is recognised by the Educational Commission for Foreign Medical Graduates (ECFMG), subject to other applicable requirements. The same notice is blunt on Afghanistan. Many Pakistani nationals are studying medicine or dentistry there. Afghan medical and dental institutions are not currently on the ECFMG-recognised list. Graduates from those institutions are therefore not eligible for the NRE and are not entitled to seek PM&DC registration to practise in Pakistan. The Council says it will not entertain exemptions, relaxations, or special dispensation. Students already there, or considering admission, are told to make an informed decision now. That is as clear a "no path" statement as a regulator usually writes. The wider licensing stack (what was already in force) ECFMG listing for NRE eligibility sits alongside rules PM&DC has been enforcing since late 2025: • Provisional registration and house jobs: from December 2025, no foreign medical or dental graduate may start a house job, internship, clinical training, or observership in Pakistan without PM&DC provisional registration. Institutions must verify status; training without it is supposed to stop. • Two tracks under the PM&DC Act 2022 (clarified September 2025): graduates of PM&DC-recognised foreign institutions may get provisional registration, then must still pass the NRE for a full licence. Graduates of institutions not yet recognised under the current law get provisional registration only after qualifying the NRE — and sitting the NRE still requires the ECFMG-list gate above. • Enrolment-stage public alert (Session 2024–25 onward): before going abroad, PM&DC expects F.Sc. Pre-Medical at 60% (or stated SAT-II / foreign MCAT-UCAT alternatives), at least 50% in Pakistan MDCAT if intermediate was completed here, an ECFMG-listed foreign institution, a free Student Registration Certificate via srf.pmdc.pk before departure, a five-year programme of at least 6,200 hours with 80% attendance, local-language preparation where English is not the medium, NRE for licensure, and a multiple entry-exit study visa covering the programme when applying for NRE. Non-compliance is stated as making a candidate ineligible to sit NRE. Who this hits hardest — and who still needs to care The July 2026 notice singles out Afghanistan. Earlier PM&DC communications also named cohorts from Central Asian republics (including Kyrgyzstan) and Iran among those prompting the foreign-graduate registration review. That is not a blanket statement that China, Hungary, Georgia, Kazakhstan, or Uzbekistan are suddenly delisted — those are still where many of our MBBS readers look. The mechanism still applies to every foreign medical graduate returning to Pakistan: ECFMG list (NRE eligibility), PM&DC recognition (provisional path), then NRE, then full licence. A university that is "recognised in the host country" is not enough. This is not a quiet, settled story Affected graduates have contested how recognition and the NRE are being run. In January 2026 the Pakistan Foreign Doctors Medical Graduates Association publicly demanded restoration of universities recognised by the World Directory of Medical Schools but delisted by PM&DC, alleged unfairness in recent NRE administration, and appealed to the Prime Minister. PM&DC's response has largely defended exam integrity and process rather than reopen eligibility. We're not adjudicating that dispute. We are also not repeating PM&DC's contested headcounts. What we can say with confidence after the 31 July notice: ECFMG listing is being treated as a hard eligibility gate, Afghanistan is explicitly closed for NRE and registration under current lists, and anyone selling "it'll be sorted later" for a non-listed school is contradicting the Council's published position. Why Green List ≠ ECFMG ≠ "you're covered" This site's Green List guidance (see our Central Asia study-options guide) is about whether a foreign university appears on PM&DC's recognition materials when you choose where to enrol. The ECFMG question is whether you can sit the NRE at all. Provisional registration and house-job rules are a third checkpoint after graduation. A consultant who says "PMDC-approved, so you're fully covered" without separating those steps is glossing over a real gap. The July notice makes the ECFMG step non-negotiable in writing. What this means if you're working with a consultant • Ask which list they checked yesterday: PM&DC recognition, ECFMG, or both — and ask for the exact institution name as it appears on each. • Ask whether they have registered a student for a PM&DC Student Registration Certificate before departure (srf.pmdc.pk), not only for university admission. • Ask if they have supported anyone through NRE and provisional registration in the last 12 months. Older "everyone gets a house job" stories do not match current enforcement. • If Afghanistan (or any non-ECFMG school) is on the table, ask them to reconcile that offer with Public Notice 1220 dated 31 July 2026. There is no soft landing in that document. Questions to ask before you commit 1. Is this exact university on the current ECFMG-recognised list — yes or no — and can you show me the listing? 2. Is it also on PM&DC's current foreign recognition / Green List materials, under the same legal name? 3. Will I obtain a PM&DC Student Registration Certificate before I leave, and who files it? 4. If lists change mid-degree, what happens to my NRE path — and has this consultant handled that situation since the 2025–26 rule changes? 5. For house jobs in Pakistan: do you understand that provisional registration is mandatory before clinical training starts? Uncertainty / subject to change ECFMG's recognised-institution list and PM&DC's own recognition materials can change.
